Medicaid Service Coordinator
College Point, Queens
Full Time: 40 Hours

Duties:

The Medicaid Service Coordinator (MSC) assist individuals with developmental and intellectual disabilities, their families and circle of support in accessing services and supports appropriate to their needs and to ensure indivuduals' needs, hopes and desires are to the best of their abilities. The MSC also provides ongoing support, advocacy and follow-up for each individual receiving case management services under the auspices of AABR, Inc. and uses a person-centered planning process in developing, implementing and maintaining an Individualized Service Plan (ISP).

 

Qualifications:

  • Associate Degree or 2 years of verifiable college education equivalent in human services or health related field
  • 1 year experience participating in assessments or service planning.
  • Working Knowledge of the OPWDD system preferred.
  • Preference given to qualified candidates who are bilingual.
  • Proficiency in Microsoft Office including Word, Outlook and Excel
  • Excellent verbal and communication skills
  • Ability to travel within the five boroughs of New York city and Nassau and Suffolk Counties.
